Pest Evaluation Form

The owner of Quality Pest Control wanted more information about a potential customer’s needs when they contacted him through their website. I designed three-step process that allowed visitors to the used images to identify which pests were a problem for them. After studying analytics for a six-month period, the pest evaluation form had a 30% higher completion level than the traditional one-step contact form on the same website.
